What's your role?
- Emphasize the importance of primary care, especially annual preventive visits - For several reasons, it’s very important for all patients, even relatively healthy ones, to have an established relationship with a primary care provider. For example, Clinically Integrated Network contract quality metrics are usually most applicable to primary care (blood pressure control, blood sugar control, etc). It’s difficult to capture and report accurate metrics if a significant portion of the population doesn’t see a primary care provider.
- Coordinate care in network – Keeping patients in the Roper St. Francis Health Alliance network is paramount to successfully managing population health. Care management intervention, clinical pathway development and other similar care redesign and improvement efforts are all for not if patients see providers and use facilities that aren’t part of the Roper St. Francis Health Alliance network.
- Get in the know – The RSF Health Alliance will succeed or fail based primarily on physician leadership. Please make sure you have a basic grasp on what’s important and why it is.
